Case Manager (1) – Client Services
A case manager at NAP helps address the on-going needs of persons living with HIV/AIDS and their family/support systems. The case manager seeks out community resources and acts as a link between them and the client, with an emphasis on linkage to medical care. The case manager is responsible for both direct and indirect contact with their clients including, but not limited to, home visits, clinical consultations, hospital visits, telephone and written communication.
Education/Work Experience/Requirements:
- Bachelors degree with 1-2 years experience. Experience in human related, i.e., social work, psychology, sociology, counseling, nursing, etc.
- Degree may be substituted by 5 years experience in the field
- Excellent verbal and written skills necessary
- Must be willing to work flexible hours
- Knowledge of and computer skills in Microsoft Office Suite
- Valid Driver’s license and insurance
