Nebraska AIDS Project provides services for over 1,000 clients and their families annually. NAP has five offices in Nebraska (Omaha, Lincoln, Norfolk, Kearney, and Scottsbluff) that serve the entire state, as well as 22 counties in southwest Iowa (Omaha office) and 3 counties in southeast Wyoming (Scottsbluff office).
Case Management
NAP’s professional case managers conduct a supportive process of assessment, advocacy, intervention, and monitoring to maintain a client’s maximum level of health, stability, and self-sufficiency. Case managers work with clients in strict confidence to facilitate links to medical and social service resources, to develop care plans, and to cultivate practical, social, and emotional support for people living with HIV and AIDS in Nebraska, southwest Iowa and eastern Wyoming.
Intake, Assessment, and Planning
Case managers start with a non-judgmental assessment of “where clients are” with respect to medical, psychosocial, financial, and daily living issues. Care plans and referrals to outside services are developed according to the client’s needs, strengths, and challenges. Clients may also consult with their case managers about sexuality concerns, drug and alcohol abuse, and mental health issues to achieve the best possible quality of life.

In addition to Case Management Services, NAP hosts support groups, client education, as well as education related to job and life skills, medication management and reducing high-risk behavior.
Lincoln, Kearney, Norfolk, and Scottsbluff Offices
These Greater Nebraska NAP offices host lunches, support groups, and other activities for clients. Schedules vary, so check with your case manager.
