Outreach

Outreach
Individuals experiencing homelessness are at a much higher risk for HIV infection.  Persons living with HIV/AIDS (PLWHA) that are homeless struggle with more barriers to care than PLWHA in the general population.  Our outreach program aims to identify HIV positive individuals who are currently out of care and link them to housing and intensive case management services at Nebraska AIDS Project.

Outreach efforts include, but are not limited to: participation in the local Continuum of Care for the Homeless, HIV testing at area emergency shelters, street outreach and agency referrals.  Once individuals are identified, Outreach staff provide intensive case management in an attempt to place the individual in stable housing and medical care, as well as assisting the individual to better utilize other available community resources.

Prevention education is provided to these individuals as well as area homeless service providers.
